The directory or file was not found, or could not be accessed. errors

I am getting  "The directory or file was not found, or could not be accessed." errors on differential backups.

Even Though

*I have created exceptions in the global exceptions

*I have created exceptions in the local exceptions

*i have created file exceptions

*i have created directory exceptions

i also noticed files don't even appear to exist, but they still cause exceptions on backup. any ideas?

Backup exec 2012 sp2 with dedupe
